Eligibility Requirements

To qualify for MyCare Ohio, you must meet all of the following requirements:

  • Age 18 or older at the time of enrollment
  • Enrolled in both Medicare and Medicaid — dual-eligible beneficiary
  • Ohio resident — must reside in one of Ohio's MyCare Ohio service regions
  • Not currently residing in a nursing facility or other institutional setting that would disqualify enrollment
  • Must meet MyCare Ohio's plan-specific enrollment criteria managed by the participating health plans
